Our office takes credit card payments and  provides super bill to submit to your insurance, as I am out of network provider for all insurances.

During your first visit you will have Dr. Strugatsky’s undivided attention, as she takes your complete health and lifestyle history, conducts a physical exam, reviews the information you provide, explains to you the approach, orders necessary studies and initiates treatment plan.

 

Payment is expected by check/cash/zelle or credit card (3% surcharge) at the time you receive your services or in advance:

• Initial visit for functional medicine evaluation $495 (1 hour 30 min in person)

• Follow up visits for functional medicine $375 (45 min), $420 (60 min)  in person or telemedicine

• Telemedicine or phone appointment $345 (30 min)
